This is a known issue that is currently sitting in limbo. It is a very weird edge case that is hard to replicate, and falls in a realm where we aren't sure if the "rule of cool" is offset by how weird continuing a round with the weirdly zoomed in camera is.
Also you lose the ability to use any items, that could be frustrating.
This happened on my very first game in the demo. Essentially an n% chance of happening every round if you have enough zombies and you are losing, with n% = whatever the chance of the trigger happening is.
I'm poked the fan with the team to raise the issue's severity a bit.
