The Paladin Dude's Medallion of Vengeance does not seem to trigger when taking fatal damage from the Lich Dude's Thrall expulsion. Given that the requirement for Medallion is "taking fatal damage from something other than his Blessing," it seems like this should trigger every time a Paladin gains Revenant. This was an intentional decision in order to prevent the Cheat Death mechanic of the Medallion from being wasted. Should it proc from that, and then the Paladin takes one solid slap from an enemy directly afterwards, he'd be unable to proc the relic again.
